The statement of offense says that, relying on the altered email, the supervisory special agent signed and submitted the application to the court on June 29, 2017. “The application for FISA #4 did not include Individual #1’s history or status with the OGA.”

That is a documented use of the writing. It is not a separate count of conviction. The guilty plea is still one count under 18 U.S.C. § 1001(a)(3) for making and using the false writing on or about June 19. The fourth application is where the government says the writing was used.

FISA #4, the same paper says, expired September 22, 2017. Conservative Treehouse later argued the timing under the special counsel. That timing argument is commentary. The dates in the plea are the facts.
